Below you will find forms you need to have filled out and submitted to participate in the Fall Middle School Tournament.
 
Remember to fill out the MIFA ML Material Certification Form. This single page replaces all those other forms you used to have the students sign. Same rule, easier paperwork.
 
The Lunch order form needs to be emailed by December 3 to dfbernar@stcs.org and then bring money to registration on Saturday morning.
 
Registration begins at 8:45 am and you should be loading back on the bus not later than 4:30 pm. Download the schedule.
 
We also included a map for your convenience.
 
Be sure to get your judge competence form in for every judge with your registration. That should be sent to mifa@themifa.org with a cc to Richard Elias relias@midasfoods.com
